From a Single Penile Suspensory Ligament to a Penile Suspensory Apparatus: Application to the Buried Penis Surgery

Abstract Goals/Purpose The ligamentous system supporting the penis is widely called « penile suspensory ligament ». It supports and attaches to abdominal wall pubic bone. However, complexity of anatomy was underestimated. aim this anatomical study describe different (features, insertion sites, path) their functions. Methods/Technique first step consisted in a description components apparatus on two cadavers embalmed by Thiel method. second part an evaluation function each freshly dead cadavers. In third stage, we applied those findings exploratory cohort patients undergoing lengthening surgery for buried penis. Results/Complications four ligaments from anterior posterior : fundiform ligament, dense vertical arcuate ligament. were loose, non-stabilizing ones stabilizing. our cohort, 12 included. mean BMI 38. pre-operative flaccid length 4.1 cm [2.2 – 7 cm] at 6 months 8.2 [5.2 - 11.5 which represents augmentation 100%. circumference state [2 post-operative 12.6 [8.3 16 85.7%. most common complication phimosis. Conclusion We have described detailed apparatus. time that brought light. To avoid instability lengthening, must be spared.
